Monte Senabung se activa en Indonesia

AW-Volcano_003

Mount Sinabung ash eruption

Mount Senabung goes live in Indonesia

Indonesia’s Mount Sinabung volcano has returned to activity by sending a hot ash cloud of up to 5 km (3.1 miles) on Tuesday 02/01, in its first major eruption since August 2020, the country’s volcano center said. Magma Indonesia. The activity of Mount Sinabung has increased since last year and the alert for the volcano in Sumatra Province has been placed at the second highest level. Mount Sinabung had been dormant for centuries before it erupted again in 2010.

Activation of the volcano has reported no casualties, but an official had previously urged people to stay at least 3 km from the crater, the Indonesian Center for Volcanology and Geological Hazard Mitigation (CVGHM) said. Videos on social media showed little panic among residents over the eruption, which sent a plume of white ash into the blue sky.

Indonesia stretches along the so-called «Pacific Ring of Fire», a highly seismically active zone, where different plates in the earth’s crust meet and create large numbers of earthquakes and volcanoes. Indonesia currently has almost 130 active volcanoes, considered the country with the highest volcanic activity in the world.

AW | 2021 03 01 20:24 | AVIATION ORGANISMS / AVIATION SAFETY

CAAC reserved on 737 MAX

China reservations about safety on the Boeing 737 MAX

China still has big safety concerns for the Boeing 737 MAX line. The Civil Aviation Administration of China (CAAC), the country’s aviation regulator, has communicated on Monday 03/01 about the considerations of other organizations to allow the plane to fly again: «Once the issues have been addressed in in its entirety, China will carry out a final overhaul of the aircraft», Deputy Chief Dong Zhiyi said at a briefing in Beijing. The CAAC aviation authority has been in full communication with The Boeing Company and the Federal Aviation Administration (FAA).

Other nations followed China’s lead on the flight ban, resulting in the Max being punished globally for about 20 months. Several major regulators have certified it to fly again in recent months and it is back in service in the United States, Europe and Brazil. Australia lifted its ban on the plane last week, as did Saudi Arabia on Monday, state media reported. But China has not moved from its position regarding approving the new corrections made by the American aerospace company.

China, a crucial market for Boeing and the 737 MAX, was the first country to impose an aircraft ban after an accident in Ethiopia in March 2019. On October 2018, a Boeing 737-8 MAX aircraft operated by Lion Air of Indonesia it plunged into the Java Sea, claiming 189 lives. Both disasters were blamed in part on the MCAS Flight Control software that pushed flight control down.

Trade tensions

Boeing’s orders in China have largely dried up in the past four years amid increased tension between Washington and Beijing over trade and other areas, as well as concerns about the MAX. «I think when a constructive relationship starts, then things will calm down and the Chinese will want to get planes», Boeing CEO David Calhoun said in December 2020, the same month the 737 MAX returned to commercial service in U.S.

CAAC criteria

China has three main criteria that must be met before allowing the 737 MAX to fly again: Design changes intended to fix the plane’s problems must be approved by China, pilots must be retrained to fly the planes after those changes, and the conclusions of the Ethiopian and Indonesian accident reports should be clear. Ethiopian investigators have said they plan to publish their final report around the second anniversary of the March 10, 2019 accident.

The three major Chinese carriers Air China, China Southern Airlines and China Eastern Airlines, along with ten other airlines are currently customers of the 737 MAX line, which will have to wait for the respective approvals from the CAAC to reestablish their flights.

The Boeing Company said that China will continue to be the main engine of global growth in the aviation industry for the next twenty years. Chinese airlines are likely to buy 8,600 new planes worth US$ 1.4 trillion during the period, the US manufacturer said.

CAAC_logo.svg

Australia aprueba al Boeing 737 MAX

AW | 2021 02 026 16:55 | AVIATION SAFETY / AVIATION ORGANISMS

Agencia aviación australiana recertifica al Boeing 737 MAX

Boeing 737 MAX_Isologotype

Australian aviation agency recertifies Boeing 737 MAX

AW-70033007

CASA_Isologotype

The Civil Aviation Safety Authority (CASA) of Australia has announced this Friday 02/26 that it lifts the ban on the Boeing 737 MAX allowing the recertification of the line to be established among the first in Asia and the Pacific. The Australian regulator has accepted comprehensive return-to-service requirements set out by the United States Federal Aviation Administration (FAA) for the 737 MAX. «We are confident that the aircraft is safe», said Graeme Crawford, Acting Head of CASA Australia, in a statement.

No Australian airline currently operates the 737 MAX, but Virgin Australia has orders for 25 units. Singapore Airlines and Fiji Airways are the ones that operated on flights to Australia prior to the model’s ground incursion in March 2019 after two plane crashes. Graeme Crawford said it was unclear when those airlines would resume flights to Australia given the COVID-19 disruption to international air travel. Singapore Airlines and Fiji Airways will also need approval to resume flight of the 737 MAX from their respective national aviation regulators and other authorities where they need to use airspace. Singapore’s aviation regulator did not immediately respond to a request for comment. Singapore Airlines said it would continue to work and be guided by regulators on 737 MAX operations. A spokesman for Fiji Airways said he was still working with other regulators in the region, including those in Fiji and New Zealand, before returning the 737 MAX to service.

CAA New Zealand

The New Zealand Civil Aviation Authority (CAA) said it had worked closely with its Australian and Singaporean counterparts upon the return of the 737 MAX in New Zealand. «The CAA will not issue blanket approval for the Boeing 737 MAX to fly to New Zealand, but will work with any future operators on a case-by-case basis to clear flight operations to New Zealand», the CAA said, noting that Fiji Airways was still restricting international flights due to COVID-19.

World regulators

Regulators in the United States, Europe, Great Britain, Canada, Brazil and the United Arab Emirates are among those who have already approved the return of the 737 MAX to flight after technical modifications and additional pilot training. Japan lifted its restrictions in late January following decisions by its counterparts in the United States, Europe, Canada and Brazil, an official with Japan’s Civil Aviation Bureau said on Friday. Currently, the Japanese country does not have local 737 MAX operators and the decision was not publicly announced at the time.

China was the first country to ban the 737 MAX from its airspace in 2019 and has not indicated when it will lift the ban. Boeing said on Friday 02/26 that it was working with regulators and customers to return the 737 MAX for safe service around the world.

Incidente vuelo FV4520 Rossiya Airlines

AW | 2021 02 26 16:16 | AVIATION SAFETY

Incident flight FV4520 Rossiya Airlines

Boeing 777-300F flight FV-4520 engine failure incident

A Boeing 777-300F, registration EI-GET of Rossiya Airlines, was carrying out cargo flight FV-4520 from Hong Kong (China) to Moscow/Sheremetyevo (Russia), it was descending towards Moscow when the crew advised that one of the FADEC channels (Full Authority Digital Engine Control), often also known as Electronic Engine Control (EEC) that controlled the left engine of the GE90 powerplant had failed. The aircraft continued normally for a safe landing on Runway 24L in Moscow/Sheremetyevo.

The plane remains on the ground in Moscow about 08:50 hours after landing. Russian and Chinese media reported that the plane was scheduled to continue to Madrid (Spain) and claimed that the crew made an emergency landing in Moscow/Sheremetyevo due to a failure in the left-hand engine, although other sources maintain that it has been the right engine and diverted to Moscow. Flight plan data shows that the plane was scheduled to land in Sheremetyevo. Radar data shows that the plane stayed in the cruise until the top of the descent at about 140nm east of Moscow and began a normal descent towards Sheremetyevo.

The pilots were diverted to Moscow/Sheremetyevo Airport after an indicator warned of a possible engine failure on the plane. The plane was being operated primarily as a cargo flight, carrying 36 metric tons of cargo and 25 people, the report said. It was unclear if the engine actually malfunctioned or the instrument light was activated incorrectly. The Russian report also did not clarify whether the pilots turned off the engine before landing.

FAA emite FBS a Boeing 737-8 200 MAX

AW | 2021 02 25 11:41 | AVIATION ORGANISMS / AVIATION SAFETY

Actualizaciones de la FAA sobre Boeing 737-8 200 MAX

faa-isologotype-3

FAA issues FBS for Boeing 737-8 200 MAX

FAA updates on Boeing 737-8 200 MAX

The Federal Aviation Administration (FAA) and the United States is proposing updates to the Flight Standardization Board (FSB) report and the Master Minimum Equipment List (MMEL) for the Boeing 737 MAX. Draft updates issued on 01/25, which are subject to public comment, primarily reflect differences between the original 737 MAX and the new Boeing 737-8 200 MAX series aircraft.

Airlines use the FSB report to develop pilot training. The new Boeing 737-8 200 MAX can carry up to 200 passengers and is equipped with an additional departure gate. The FAA has proposed additional training to account for the design changes associated with these features. The FSB draft report also includes minor revisions that are part of the agency’s ongoing safety oversight. None of the proposed changes would affect the training curriculum that the FAA approved as part of the 737 MAX return to service.

The MMEL details the circumstances in which airlines can still operate flights without certain systems or components. The proposed MMEL adds new relief for items that were identified through the agency’s ongoing interactions with operators and the manufacturer. None of the proposed revisions involves the aircraft’s automated flight control system, which underwent a review and certification 20 months after the fatal accidents of Lion Air flight JT-610 and Ethiopian Airlines flight ET-302.

The FAA has accepted comments on the draft FSB report until February 16, 2021. Comments on the draft MMEL were accepted until February 24, 2021. The FAA will release final versions of both documents after the agency reviews and addresses those comments.

Informe DOT procesos de la FAA

AW | 2021 02 25 10:05 | AVIATION SAFETY / GOVERNMENT

DOT Report FAA processes

FAA initial defective Certification Process line 737 MAX

The Inspector General of the United States Department of Transportation (DOT) has expressed in a report that identifies weaknesses in the review of the initial processes for the certification of the Boeing 737 MAX passenger aircraft by the Federal Aviation Administration (FAA).

Following the accidents involving Boeing 737 MAX aircraft, they led to the global grounding of the fourth-generation 737 line for a period of twenty months. Both air crashes were largely caused by the MCAS System, a new technology on the 737 MAX line that was designed to push the nose of the aircraft down during certain flight conditions necessary for a correction. Because the new model has larger engines where the aircraft’s Center of Gravity (MAC) has been modified, a compensation adjustment with the MCAS System has been necessary. After ordering a series of modifications to the MCAS Flight Control System and further pilot training, where initially the flight crews received little guidance on interacting with MCAS, the FAA lifted its grounding order in November 2020. The aircraft has returned to service in Brazil, Canada, the United States, the European Union, the United Kingdom, among other countries such as Nigeria and Kenya.

DOT report

The Federal Aviation Administration’s process for aeronautical certifications is back under scrutiny after the Department of Transportation’s Office of the Inspector General said on Wednesday 02/24 that the agency needs to strengthen its aircraft review process. Although the 55-page report states that the FAA did not deviate from an established protocol when it first cleared the plane for flight in 2016, it significantly misinterpreted the Flight Control System called Maneuvering Characteristics Augmentation System (MCAS) which was flawed and determinative. in the failures of the flights JT-610 of Lion Air and ET-302 of Ethioopian Airlines in which a total of 346 people died.

The DOT report details: «The FAA certification guide does not adequately address the integration of new technologies into existing aircraft models. Second, the FAA did not have a complete understanding of Boeing’s safety assessments conducted in the MCAS system until after the first accident». The DOT report also found that the FAA’s Boeing Office of Aviation Safety Oversight had limited resources with only 47 employees and recommended changes to a program called Organization Designation Authorization (ODA) in which the FAA delegates parts of its aircraft certification process to third party companies. Boeing used the ODA to certify the 737 MAX and earlier aircraft like the 787 and 777, meaning it inspected its own aircraft under the guidance of the FAA. «We confirmed in interviews with representatives of the FAA and Boeing ODA that there were instances where the same company engineer worked on a particular design and then approved the design as a member of the ODA unit. This may not provide sufficient independence. and could cause a conflict of duties for those unit members», the DOT document states.

The definitions primarily echo the conclusion of an independent panel that the DOT created in January 2020 to study how the 737 MAX was first certified. Although that committee said the FAA could improve the certification process, it saw no need for substantial changes. «The Committee found the FAA certification system to be effective and a significant contributor to the world’s safest aviation system. The [FAA] is well aware of the challenges to safety in the midst of a rapidly evolving and expanding industry», says the executive summary of the independent panel of the DOT Inspection Committee. At the end of the report on Wednesday 02/24, the Office of the Inspector General made 14 recommendations, including reviewing the requirements of the ODA Program to prevent any interference in the certification processes, strengthening communication between all parties involved and the improvement of protocols for the review of new technologies. The report has also included a statement from the FAA, which promised to implement all the recommendations. «The FAA agrees with the OGA’s recommendations, many of which align closely with those of other reviews and experts, including the Secretary’s Special Committee to review the FAA Aircraft Certification Process», wrote H. Clayton Foushee, Director of the FAA’s Office of Audit and Evaluation.

The Boeing Company said in a separate statement on Wednesday 02/24 that it fully and extensively cooperated with the review. «We appreciate the Inspector General’s review of the 737 MAX design and certification, as well as recommendations to strengthen and improve the regulatory process».

ONU condena atentado PS-752

AW | 2021 02 24 14:14 | AIR INVESTIGATION / AVIATION SAFETY

Derechos Humanos acusa Irán por derribo vuelo Ukraine Airlines

UN_Human_Rights_Council_Isologotype

Human Rights accuses Iran of Ukraine Airlines flight down

OHCHR_Isologotype

The United Nations Human Rights Council accuses the Government of the Islamic Republic of Iran of violating international norms and human rights around the downing of a Boeing 737-800 flight PS-752 of Ukraine International Airlines in January 2020. In a letter to Iran, two UN experts argue that Iran has made many mistakes in shooting down Ukrainian civilian aircraft, according to the UN.

Iran’s Revolutionary Guard shot down a passenger plane that had just taken off from Tehran Airport on January 8, 2020, killing the 176 occupants of flight PS-752. According to the Iranian government, the members of the guard thought it was a US missile and had to act quickly. However, UN experts say the Iranian authorities have told an incoherent story. Other passenger planes taking off in front of the Ukrainian plane have not been shot down. Furthermore, based on its own investigation, the UN reports that the missile system did not have adequate precautions, that the people who fired the missile had much more time to make a good decision than Iran says, and that they should have known that a plane had just taken off. Experts write that Iran should have closed its airspace due to the tense situation between Iran and the United States. «If that had happened, 176 innocent people would still be alive today», maintains the Human Rights Council.

After the plane was shot down, the Government of Iran should have recognized earlier that the plane had been shot down by a missile, according to the UN. The Government did not do that for three days. By then, the disaster site had already been completely looted and flattened by bulldozers. «For example, Iran has deprived the bereaved of the opportunity to retrieve objects from those closest to them», the UN experts said. The actions of the Government of Iran have been systematically repudiated for the gross violations of human rights showing the lowest human miseries.

FAA emite orden AD para PW4000

AW | 2021 02 24 09:50 | AVIATION SAFETY / AVIATION ORGANISMS

FAA ordena inspecciones inmediatas motores PW4000 Boeing 777

FAA-Isologotype

FAA issues AD order for PW4000

FAA orders immediate inspections of PW4000 Boeing 777 engines

The Federal Aviation Administration (FAA) announced this Tuesday 02/23 that it was ordering immediate inspections of Boeing 777-200 aircraft with Pratt & Whitney PW4000 engines before new flights after an engine failed on United Airlines flight UA328 on Saturday 02/20. The FAA spent the past two days discussing the scope of the inspection requirements, according to sources with knowledge of the matter.

The FAA has ordered through an Airworthiness Directive (AD) 2021-05-51 the immediate order of the inspections of the PW4000 engines of the fleet of Boeing 777-200/300 aircraft existing in the airlines in the world. Operators are required to conduct a thermal acoustic imaging inspection of the large titanium fan blades located at the front of each engine, the FAA said.

The National Transportation Safety Board (NTSB) reported Monday 02/22 that a cracked fan blade from the engine of United Flight UA328 that caught fire was consistent with metal fatigue. «Based on the initial results as we receive them, as well as other data obtained from the ongoing investigation, the FAA may revise this directive to establish a new interval for this inspection or subsequent ones», the FAA said.

The damaged starboard engine of United Airlines Flight UA328 is seen after the incident as a repeat incident. In March 2019, after an engine failure on another 2018 United Airlines flight attributed to fan blade fatigue, the FAA ordered inspections every 6,500 cycles (Cycle: is a take off and landing). The US FAA said in 2019 that each inspection was expected to take 22 man-hours and cost $ 1,870.

A spokeswoman for Pratt & Whitney, owned by Raytheon Technologies, said the fan blades would have to be sent to its repair station in East Hartford, Connecticut, for the latest inspections, including those of airlines in Japan and South Korea.

The South Korean Ministry of Transport has ruled this Tuesday 02/22 that it had told its airlines to inspect fan blades every 1,000 cycles following the Pratt & Whitney guide after the latest United Airlines incident. An airline would typically accumulate 1,000 cycles every 10 months on a Boeing 777, according to an industry source familiar with the matter.

Japan’s Transport Ministry said on Wednesday 02/24, through the Japan Civil Aviation Bureau (JCAB) that it was reviewing the FAA’s Airworthiness Directive (AD) and had not yet decided what action to take. ANA Holdings and Japan Airlines (JAL) said they would comply with any directives from the Japanese regulator.

United UA328

The PW4000 engine that failed in the 26-year-old Boeing 777-200 and broke loose over a Denver suburb on Saturday 02/20 has been a serious warning about structural limits and inspection planning processes due to fatigue. of material found recurrently in more momtores. United has warned of possible disruptions to its cargo flight schedule in March 2021, rescheduling its fleet after its decision to ground 24 Boeing 777-200s. Another 28 Boeing 777-200s were grounded before the incident on Saturday 02/20, amid a drop in demand.

Currently, PW4000 engines are used in 128 aircraft, representing about 10% of the global fleet of more than 1,600 aircraft delivered. United Airlines is the only American operator with a fleet of Boeing 777s equipped with PW4000 engines, it had temporarily grounded its fleet prior to the FAA announcement. The Boeing Company had recommended that airlines suspend use of the planes, while the FAA identified an appropriate inspection protocol.

Nigeria aprueba recertificación 737 MAX

AW | 2021 02 22 14:02 | AVIATION SAFETY / AVIATION ORGANISMS

Agencia Aviación NCAA levanta prohibición operaciones B737 MAX

AW-790001

Nigeria passes 737 MAX recertification

NCAA Aviation Agency lifts ban on B737 MAX operations

The Nigerian Civil Aviation Authority (NCAA) has approved the recertification of the Boeing 737 MAX line to operate in Nigerian airspace. With this approval, foreign airlines could now fly the plane in the African country’s airspace, while the local airline, Air Peace, could also receive their formal orders for the Boeing 737 MAX. In a Memorandum to Operators dated February 12, 2021, the NCAA stated that the Boeing series was approved with a new operational recertification for local and foreign airlines.

The 737 MAX had been on the ground globally for almost two years after two plane crashes where investigations have identified problems in the manufacturing line with respect to the design of the Flight Control System known as MCAS. The Minister of Aviation of the Republic of Nigeria, Hadi Sirika, had also banned the Boeing 737 MAX aircraft from operating in Nigerian airspace. The FAA gave its approval to the latest version of the 737 model to resume passenger flights, while the Department of Justice accused The Boeing Company of conspiracy to defraud the United States and would have to pay a fine of US$ 2.5 billion for lying to the FAA before and after the fatal accidents involving Lion Air flights JT-610 (10/2019) and Ethiopian Airlines ET-302 (03/2019).

AW-773377

The NCAA noted that on November 18, 2020, it received a Continuous Airworthiness Notification from the International Community (CANIC) CAN-2020-24 informing it of the continuing operational safety activities of the FAA related to the return to service of Boeing 737 aircraft. 8 MAX and 737-9 MAX. This, however, prompted the FAA to issue a final rule, the Airworthiness Directive (AD), which mandated actions for Boeing 737 MAX aircraft, including the installation of new Flight Control computer software and new System software. of the 737 MAX, the incorporation of certain flight crew operating procedures from the Flight Manual, the modification of the horizontal stabilizer adjustment cable routing facilities, the steering angle of the Attack and Operation Preparation Flight.

NCAA Director General Musa Nuhu said he recognized that a Joint Authority Technical Review (JATR) made up of international aviation authorities such as the European Aviation Safety Agency (EASA), Transport Canada (TC) and Civil Aviation The Authority of Singapore (CAAS), among others, conducted a joint review of the Boeing 737 MAX safety system with the FAA and NASA. «In light of the above, the FAA has released documents on the Boeing 737 Flight Standardization Board report, Revision 17, which identifies special pilot training for the 737 MAX and the Operator Safety Alert. NCAA recognizes the joint review of the Boeing 737 MAX Safety System and allows the necessary actions for all foreign and domestic operators. The entire intention of domestic operators is to work with Boeing and the NCAA so that the Type Certificate Acceptance Program of Aircraft has the Boeing 737 Max aircraft registered in Nigeria and issues a Standard Certificate of Airworthiness», said the aviation agency NCAA.
