AW | 2018 11 24 23:37 | AIR INVESTIGATION / AVIATION SAFETY

Lion Air JT-610 | Day 27

The search for 64 remaining victims and the CVR will be resumed

Lion Air plans to resume the search for the 64 victims of accidents that remain unidentified from the 189 people on board of flight JT-610. The head of the Medical and Health Center of the National Police, Brigadier General Arthur Tampi, said the team had identified 125 victims. «There are still 64 victims who have not been identified», said Arthur Tampi of the Kramat Jati Police Hospital.

During the 13-day search, the National Search and Rescue Agency (Basarnas) recovered 666 body parts, which were then examined by the identification team of disaster victims for identification, Tempo reported. It was suspected that the victims could not be identified since the bodies were still missing, so Lion Air planned to resume the search. «There are signs of a new search, we will be coordinating with the head of Basarnas», said Daniel Putut Kuncoro, Managing Director of the Lion Group, at Kramat Jati Police Hospital. The head of the DNA Laboratory of the Medical and Health Center of the National Police, Commander Putut Cahyo Widodo, said that although the search had been suspended, his team was destined to provide assistance in case more bodies of victims were found. The identification process will be longer because the bodies must have deteriorated. Divers are still looking for the cockpit voice recorder (CVR).

Lion Air has said that it is paying a little more than US $ 100,000 (S $ 138,000) in compensation to the families of each victim of the accident.

So far, researchers have said that the Boeing 737-8 MAX aircraft had problems with its Air Speed Indicator and the Attack Angle Sensors (AOA), which has pushed Boeing to issue a Special Bulletin indicating that the operators what to do when faced with the same situation. An AOA sensor provides data on the angle at which the wind passes over the wings and tells pilots how much an airplane goes up. The information can be critical to prevent the plane from stopping.

Last week, a union of US airline pilots APA said that Boeing had not informed the companies and pilots about certain changes in the control system of the aircraft. While Boeing has been criticized for possible failures in its latest 737 model, launched last year, the accident also resurrected concerns about Indonesia’s poor safety record, which until recently saw its airlines facing airspace in the European Union and the United States.

Next Wednesday, November 28, 2018, the Indonesian Air Safety Committee (KNKT) must submit a formal Preliminary Report on the causes of the accident.

Cabin Safety

Flight data does not tell the full story. The Cab Flight Recorder (CVR) has not yet been recovered, which would help indicate what the pilots were thinking about the malfunctions. The situation of instruments and alarms in conflict could have been confusing, although there are more complicated situations in other investigations.

It is reasonable to ask why the pilots did not turn off the adjustment system during the approximately 10 minutes that evaluated the behavior of the aircraft. A decades-old emergency procedure teaches pilots to turn on two switches to cut off the engine power that pushes the nose down. Even more disconcerting is the reason why the plane exhibited the same faults that it had on the previous flight after it was supposedly repaired during the night.

On a previous flight from Bali-Jakarta on 28 October 2018, the airplane shaker on the captain’s side was active for almost the entire flight of approximately 90 minutes and the Attack Angle (AOA) readings had the same errors, according to the data. With this in mind, there are questions that need to be answered. A \ W

No hubo heridos.

No hubo fatalidades.

AW-70073780New incidents in Boeing 737 MAX and demands of relatives of flight JT-610

The Boeing 737 MAX is on the crest of the wave facing serious safety problems as investigations continue of the air crash of Lion Air‘s JT-610 flight. New incidents occurred these days with the same model, fortunately without major incidents. Relatives of the victims of flight JT-610 initiate lawsuits in the United States.

Faults in other Boeing 737 MAX

According to the Transportation Safety Board of Canada (NTSB), the crew of a Boeing 737-8 MAX from Sunwing Airlines received erroneous information while flying at about 35,000 feet when it was flying WG-439 between the International Airports of Punta Cana-Toronto , last 14 November 2018.

The Canadian Sunwing Airlines joins the airlines that in recent days have detected failures in the Boeing 737 MAX, the aircraft model that crashed on 29 October 2018 in Indonesia and belonged to Lion Air.

Family demands

Relatives of the 189 fatalities of the JT-610 flight of the Boeing 737-8 MAX of the Indonesian low cost airline Lion Air that crashed in the Java Sea on October 29, 2018, have filed more lawsuits against the US manufacturer Boeing in a Court of the United States, their lawyers reported today. The law firm Wisner Law Firm and Ribbeck Law Chartered independently requested the start of litigation against the aerospace multinational, in addition to the lawsuit filed last week by the father of one of the deceased in the incident. The lawyer Floyd Wisner, Wisner Law Firm, said in a telephone conversation from Chicago (USA) that so far represent three families of victims in a court of the State of Illinois, where is the headquarters of Boeing. For his part, the lawyer Manuel von Ribbeck, of Ribbeck Law Chartered, said that the relatives he represents could receive hundreds of millions of dollars.

The plaintiffs allege security problems in the damaged model, the Boeing 737-8 MAX, which were not communicated by Boeing to the airlines and pilots that use this aircraft.

The flight data of the first black box (FDR) of the crashed device revealed failures in the new automatic system to prevent the 737 MAX models from stalling, ie when the plane does not have enough speed to stay in the air. The detected failure can cause in certain circumstances that the nose of the plane descends of excessive form. «It was probably a defect of the Boeing aircraft that has resulted in an unimaginable anguish for the families of the victims, not to mention the enormous economic burden that will befall many», Wisner said in a statement.

Position of the FAA

The Federal Aviation Administration (FAA) of the United States qualified this condition of the recent Boeing 737 MAX models of insecure in a directive on November 7, 2018.

Boeing, in a statement on Wednesday, defended the safety of the Boeing 737 MAX and reported that it has sent two operational updates to airlines to solve the potential problem. In the last week, Boeing has stepped up its response by rejecting suggestions that the company could have better alerted its customers about the aircraft’s new anti-lock function.

Airlines with Boeing 737 MAX aircraft in their fleets and pending orders have been eager to obtain information about the little-known anti-merchandise feature of the Max that has become a focus area.

Next Preliminary Report

The National Committee of Security in the Indonesian Transport (KNKT) will present on November 28, 2018 the Preliminary Report on the causes of the accident of flight JT-610 of Lion Air. With the voice recorder in the cockpit of the Lion Air aircraft still difficult to reach almost a month after the disaster, Indonesian researchers rely on the flight data recorder, interviews with technicians and pilots for a preliminary report to be delivered to the next week.

The first findings of Indonesia’s National Transport Security Committee can be published on November 28, President Soerjanto Tjahjono told lawmakers in Jakarta. Indonesian researchers have indicated that a faulty system may have caused the nation’s worst aeronautical disaster in two decades, prompting Boeing to strengthen its pilot manual for airlines operating with the latest generation of its best-selling model.

The Preliminary Report will contain the possible causes of the fatal immersion of the Boeing 737-8 MAX in the Java Sea off Jakarta with 189 people on board. The report will detail the facts and the description of what the investigators have learned from the flight data. A missing piece of the investigation has been the cockpit voice recorder that contains the conversation between the pilots and the air traffic controller, which is still not found.

Failures in Lion Air

The fearful behind the scenes of a low cost airline that has ventured into many problems with its fleet of aircraft has weathered different pressures from spending the minimum on the flight path, pilots with 24-hour shifts, broken pieces passing from a plane to another, I enter other details.

Makassar flight case
Several years ago, the Indonesian Government’s security inspector had spent the entire night at Makassar Airport in eastern Indonesia, carefully analyzing a Lion Air aircraft that had suffered a hydraulic failure. Once he told the airline’s employees that the plane had to remain on the ground until the problem was solved, the inspector returned to the hotel to take a shower. When the inspector returned to the airport, the plane was on the runway about to take off. Furious, he ordered the passengers to disembark. However, a Lion Air supervisor explained how the airline had gone over his head: federal transport officials in Jakarta, the capital of Indonesia, authorized him to take off. A few minutes later, the plane was in flight.

The famous safety record of Lion Air, Indonesia’s largest air carrier and one of the fastest growing airlines in the world, is once again at the center of attention after the plane crash of flight JT-610, which It crashed in Indonesian waters with 189 passengers on board, a few minutes after taking off on October 29, 2018.

Investigations on Lion Air

The researchers try to find out what was the deadly alchemy of factors that caused the fall of the new Boeing to more than 600 kph. They are analyzing whether Boeing did not adequately explain the modifications made to the Boeing 737-8 MAX aircraft; how Lion Air handled the repeated failures in the readings of data of the airplane before the accident; and how pilot training or confusion may have come into play in a case where only a few available seconds would have been enough to save the plane and its occupants. But even while the mystery of flight JT-610 is still being analyzed, something is clear, researchers and aviation experts say: few airlines were less prepared for the crisis than Lion Air.

Interviews with dozens of Lion Air management personnel and flight crew members and on the ground, as well as with Indonesian researchers and airline analysts, describe a picture of an airline so obsessed with growth that it did not build a correct safety culture.

Given that Lion Air Group owns several air transport lines, including Lion Air, and expands strongly in both domestic and foreign flights, new questions are raised about the company’s astounding rise. Lion Air is ranked among Indonesia’s highest-profile companies, although it remains overshadowed by the dark stories and incompetence of former employees and industry regulators.

Even though Lion Air Group signed the two largest contracts in the history of aviation in recent years, its flagship carrier suffered at least 15 major security failures, including a plane crash that killed 25 people, and hundreds of episodes. more than they have escaped public attention, say aviation experts.

Government security researchers say that the company’s political ties have allowed it to evade its recommendations, as happened in the episode in Makassar, and downplay instances that would have caused alarm elsewhere. Lion Air became adept at passing failing equipment from one plane to another, rather than solving problems, former company employees said.

Lion Air did not respond to repeated requests for comments regarding specific instances in which former employees and government investigators claimed that the company had breached security standards.

Frank Caron, Lion Air’s Security Manager from 2009 to 2011, who was called at the request of the insurance companies, said the airline had an average of one major engineering failure every three days, even though most of its Fleet was new. «Buy all the innovative last generation engineering will be in vain if the systems that prioritize safety are not implemented», he said. Caron said that in his first month on Lion Air, insurance companies were shown daily flights that drastically declared fewer hours worked by pilots. «What I saw was a company, from top to bottom, that made money saving, their motto, therefore, they spent the least on pilots’ training, salaries, management, everything», Caron said.

Edward Sirait, Chairman of the Lion Air Group, denied that the company saved or hid anything in the flight diaries. In an interview in his barely furnished office, he said the company had twin priorities: growth and security. «When we expand, we think of all the markets we have to reach. Although we always develop in accordance with our fleet, human resources, crew and maintenance facilities. The Lion Air pilots were professionals and would not carry double flight diaries. If anyone were discovered doing that, the license would be revoked», he said.

Political power

Lion Air, Indonesia’s first low-cost private airline, was founded in 1999 by Rusdi Kirana, a former typist, salesman and pastry chef whose only experience in the aviation industry was having a travel agency.

Currently Lion Air Group is the largest air transport line in Southeast Asia in terms of the size of its fleet. The airline has 458 aircraft in formal orders throughout the air group. To supply some of its 30,000 workers, the company built its own neighborhood on the outskirts of Jakarta, called Lion City.

From the beginning of the airline, Lion Air was surrounded by secrets. Lion Air Group released few financial details. He paid all those planes asking for very bulky loans from foreign banks and aircraft leasing companies. Lion gathered so many creditors that some banks were cautious, even before the accident of flight JT-610. «Many banks have exposure or overexposure to them and are reluctant to lend more money», said David Yu, Managing Director of Inception Aviation Holdings, a European aircraft rental and investment company.

The airline tried to approach its political connections. By 2014, Kirana had risen to the vice presidency of the National Awakening Party, the largest Islamic political party in the nation with the largest Muslim majority in the world. It was a curious position for a businessman of Chinese and Christian ethnicity. Kirana, who refused to be interviewed, was economic adviser to President Joko Widodo of Indonesia. Last year, Kirama successfully lobbied to become Indonesian Ambassador to the neighboring Republic of Malaysia, where Lion Air Group launched into competition with AirAsia, the region’s other large low-cost airline.

Edward Sirait, President of Lion Air Group said: «The successful geography of Indonesia between India and China shaped the ambitions of Lion Air, we look at the radius between China and South Asia, and it will continue to expand with extraordinary economic growth. That’s our dream, that’s why we buy the aircraft».

For aircraft manufacturers such as Boeing and Airbus, low-cost airlines in the developing world are booming, despite concerns about lax safety standards. «This is an example of a win-win situation where the people of the region can benefit from an outstanding airline», President Barack Obama said in 2011 when the Lion Air Group signed an order for US$ 22 billion. dollars for Boeing aircraft, the last largest single order in the manufacturer’s history. However, despite all the ribbon celebrations and cuts, aviation experts worried about the company’s too-rapid growth for its own sake.

Long history of incidents

Members of their flight and maintenance crews, speaking on condition of anonymity, to avoid losing their jobs, say they were pressured to carry double flight books to hide excessive work and lack of attention to safety. Pilots say they resorted to the use of methamphetamine to survive strenuous schedules.

Over the years, Lion Air aircraft collided with a cow, a pig and what is worse, each other. Two days in a row in 2011, Lion Air planes ran off on the same runway at an airport.

In 2013, a Lion Air flight landed at sea at Bali Airport. The official reports of the accidents accused the first official of 24 years of lacking the basic principles of aircraft flight and advised Lion Air to ensure that the pilots are properly trained.

«There are so many bad stories about Lion Air, it’s hard to know where to start», said Ruth Simatupang, a former researcher for the National Transport Security Committee of Indonesia (KNKT).

Since the days of the Lion Air Group flight JT-610 accident, Lion Air Group aircraft have been involved in two more episodes: one in which the wing of an airplane hooked an electricity pole and another in which a plane had a hydraulic failure.

«Everything that is wrong in Indonesia can be found in Lion Air. Do we want this company to represent us, making us look like another Third World disaster?, said Alvin Lie, Indonesian official Ombudsman and aviation expert.

AW-700066666Signs of danger

Captain Hasan Basri, pilot of Lion Air, said that two years ago, he checked the flight log and found that the weather radar on the nose of the plane he was going to pilot, did not work. The problem should have been repaired within 10 days. But Hasan said the airline had the habit of simply passing the failed radar to another plane.

Not being able to depend on the aircraft’s hardware caused unnecessary stress on the pilots, who had already flown extra hours, according to former Lion Air pilots. The twenty-four hour shifts were not unusual.

Lion Air pilots are hired for two years, a questionable practice under Indonesian labor laws, and they must pay the company large fines if they decide to leave the company.

Even under its own administration, Lion Air has spared the training of pilots compared to other airlines. When the pilots of Garuda, the national airline of Indonesia, trains to fly the Boeing 737-8 MAX, the same new model that had an accident last month, they travel to Singapore to practice in simulator. The pilots of Lion Air, on the contrary, make an online learning program of three hours.

«For the aviation industry, safety must be a priority. But the way in which they treat pilots and maintenance crews, the extra work and the fatigue and worries about airline mismanagement, create an insecure environment», Hasan Basri said.

For two days before its final flight, the Lion Air plane that landed in the Java Sea recorded inaccurate data readings. Each one of them by itself could have seemed surmountable. But, as the anomalies accumulated, the plane was still flying. «As long as the priority is to put the planes to fly, instead of security, then they will have problems», said Simatupang, a former government researcher.

Laura Lazarus became a flight attendant for Lion Air at age 19, after a month of training even when it was supposed to be three months, she said she flew up to six routes a day. In 2004, Lazarus said she was involved in her first accident, when a plane left the runway in the city of Palembang. Four months later, the landing of a plane in Surakarta again went off track and entered a cemetery, killing 25 people. Lazarus had a broken arm, leg and hip. The accident also ripped part of his calf. For years, he fought for his compensation, but says he got tired of fighting. «I have no more tears to shed because of the way Lion Air treated me. It is better to leave everything in the past», said Laura Lazarus.

Black aviation list of the European Union

The list of airlines banned in the European Union were involved Lion Air, Wings Air and Batik Air, which were one of several Indonesian carriers that were banned from operating in European airspace, due to the concerns of the European Commission about the capacity of the Directorate General of Civil Aviation of Indonesia (DGCA) to provide adequate regulatory oversight of the aerial industry. On June 16, 2016 the restrictions on Lion Air were lifted to be able to fly to Europe.

Incidents and accidents

AW-Boeing 737-8MAX.png

On January 14, 2002, Lion Air’s JT-386 flight, a Boeing 737-200 crashed after attempting to take off with an incorrect flap configuration at the Sultan Syarif Kasim II International Airport. All aboard survived, but the plane was canceled.

On November 30, 2004, flight 538 of Lion Air, a McDonnell Douglas MD-82, crashed in Surakarta with the registration PK-LMN (C / N 49189); 25 people died.

On March 4, 2006, Lion Air JT-8987, a McDonnell Douglas MD-82, crashed after landing at the Juanda International Airport. The reverse thrust was used during the landing, although it was said that the left thrust reverser was out of service. This caused the aircraft to deviate to the right and exit the runway, remaining at rest at about 7,000 feet (2,100 m) from the approach end of the runway. There were no fatalities, but the aircraft was badly damaged and then it was canceled.

On December 24, 2006, Lion Air’s JT-792 flight, a Boeing 737-400, landed with an incorrect fin configuration and was not aligned with the runway. The plane landed hard and slid along the runway, causing the right main landing gear to fall off, the left train to protrude from the wing and part of the fuselage of the aircraft to be wrinkled. There were no fatalities, but the plane was canceled.

On February 23, 2009, JT-972 flight of Lion Air, a McDonnell Douglas MD-82, landed nose-first at the Hang Nadim International Airport, Batam.

On March 9, 2009, the JT-793 flight of Lion Air, a McDonnell Douglas MD-90-30 (registration PK-LIL) left the runway at the Soekarno-Hatta International Airport. Nobody was hurt.

On November 2, 2010, Lion Air’s JT-712 flight, a Boeing 737-400 (with PK-LIQ registration) surpassed the runway when landing at the Supadio airport, Pontianak, resting on its belly and suffering nose damage. The 174 passengers and crew were evacuated through the emergency slides, with few injuries.

On April 13, 2013, the JT-904 flight of Lion Air, a Boeing 737-800 (registration PK-LKS; C / N 38728) from Bandung to Denpasar with 108 people on board, crashed into the water near Denpasar / Bali while trying to land. The fuselage of the plane broke into two parts. While Indonesian officials reported that the aircraft crashed before the runway, reporters and photographers from Reuters and Associated Press indicated that the plane had passed the runway. All passengers and crew were evacuated from the aircraft and there were no fatalities.

On August 6, 2013, the JT-892 flight of Lion Air, a Boeing 737-800 (registration PK-LKH; C / N 37297) on the Makassar-Gorontalo route with 117 passengers and crew on board, hit a cow while it landed in Jalaluddin airport and deviated from the track. There were no injuries.

On February 1, 2014, Lion Air flight JT-361, a Boeing 737-900ER (registration PK-LFH; C / N 35710), from Balikpapan Sultan Aji Muhammad Sulaiman Airport to Ngurah Rai International Airport in Denpasar / Bali through Juanda International Airport in Surabaya, with 222 passengers and crew on board, landed hard and bounced four times on the runway, causing a tail blow and substantial damage to the plane. There were no fatalities, but two passengers were seriously injured and three others suffered minor injuries.

On February 20, 2016, Lion Air JT-263 flight from Balikpapan Sultan Aji Muhammad Sulaiman Airport to Juanda International Airport in Surabaya passed through the runway upon landing, without injury. The National Transportation Security Committee’s investigation into the incident found that flaws in the management of crew resources led to incorrect landing procedures and recommended that Indonesian airlines improve pilot training.

On April 2, 2017, about 300 liters of fuel were spilled on the runway of the Juanda International Airport in Surabaya. The photos taken by the passengers on board showed fuel coming from one of the wings of the aircraft. Soon after, all passengers were evacuated and the plane was grounded for further investigation. No victims were reported. That same day, a Lion Air representative was summoned by the Ministry of Transport of Indonesia to clarify the incident. An early statement from a representative of Lion Air said the leak was caused by a safety valve that does not work and an overflow detector.

On April 29, 2018, JT-892 flight of Lion Air, a 737-800 (registration PK-LOO), made an excursion at Jalaluddin Airport after landing in conditions of heavy rain, which caused the collapse of the main train . There were no fatalities.

On October 29, 2018, Lion Air’s JT-610 flight, a Boeing 737-8 MAX, (PK-LPQ registration) crashed into the Java Sea 13 minutes after Jakarta’s takeoff, killing the 189 people on board.

On November 8, 2018, the JT-633 flight of Lion Air, a Boeing 737-900ER rolled to depart at the Fatmawati Soekarno Airport when its left wing hit a light pole, severely damaging the front edge of the aircraft.

November 2018, a flight of a Boeing 737 of Lion Air from the Republic of Malaysia suffered a hydraulic failure when arriving in Jakarta, Indonesia (Without Full Data). A\W

The National Transport Safety Committee (Komite Nasional Keselamatan Transportasi, KNKT) will announce the results of an investigation into the black box of the Lion Air JT-610 aircraft later this month.

This was declared by the President of the KNKT, Soerjanto Tjahjono, at a joint working meeting of the V Commission of the House of Representatives (DPR) in the parliament building on Thursday 22 November 2018. «The black boxes will be announced on the 28th, according to the fact that 30 days after the event an initial report will be presented«, said Soerjanto. According to the NTSC analysis of the black box data, there are indications that the aircraft lost the lift because the steering did not work optimally. even if it stays in the air. The results of the NTSC analysis also indicated that the situation that occurred on the Jakarta-Pangkal Pinang flight had previously experienced similar control on the Bali-Jakarta flight.

Soerjanto Tjahjono said that at the end of this month he will announce immediately in detail the results of the analysis made of the black box of the Boeing 737-8 MAX flight JT-610 of Lion Air. «On the 28th, the NTSC will issue a preliminary report», he explained. In addition to delivering the results of the Black Box analysis, the NTSC on the same date will also transmit the results of the Flight Data Recorder (FDR) analysis.

CVR, even without appearing

The report will detail the facts and the description of what the investigators have learned from the flight data. A missing piece of the investigation has been the Voice of the Cabinet Recorder (CVR) that contains the conversation between the pilots and the air traffic controller. JT-610 flight of Lion Air crashed a few minutes after taking off into the shallow waters of the sea.

With the Cabin Voice Recorder (CVR) of the Lion Air pilot still difficult to reach nearly a month after the disaster, Indonesian investigators rely on the flight data recorder, interviews with technicians and pilots for a preliminary report that will be delivered next week.

The KNKT said the agency was still trying to locate the so-called attack sensor angle, while the search of the cockpit voice recorder is underway. An underwater pipeline passing near the accident site and the lack of equipment to sweep the seabed make it difficult to search the CVR. A \ W

AW | 2018 11 22 09:40 | AVIATION SAFETY / AIR TRAFFIC

Aviation Alert Veniaminof Volcano, Alaska

Red Warning for aviation Veniaminof Volcano throws ashes at 15,000 feet

The activity of the Veniaminof Volcano, a volcano on the Alaska Peninsula, has intensified significantly, throwing ashes at 15,000 feet in the air, prompting the United States Geological Survey to increase the warning of the Red Alert code for aviation.

Continuous emissions from the Veniaminof Volcano had continued for much of today, with a boom of less than 15,000 feet extending 250 miles southeast to approximately 2.25 AKSY. A brief eruptive pulse was observed from the Perryville webcam at 1.26AKST but dissipated largely with 17.26 AKST.

The volcanic ash warning from the National Weather Service for Perryville remains in effect at night due to prevailing winds during the night if ash emissions occur. Ash fall is possible in nearby communities. The Anchorage Forecast Office of the National Weather Service will issue a warning if this is expected.

The National Meteorological Service has also issued a SIGMET warning for aviation. Veniaminof, which ranks 29th on the recent list of the most dangerous volcanoes in the United States, according to USGS, has been in eruption since September 4, 2018, but activity has increased significantly on the last day.

Alaska and the Aleutian Islands chain is one of the most active volcanic areas in the world, located in the so-called Ring of Fire. Strong eruptions of explosives at Veniaminof occurred in 1939, 1956 and, most notably, in 1983, where ashes were thrown 25,000 feet into the air. It was the site of a massive eruption in about 1750BC, which left a large caldera.

The most recent eruptions have all been of the ash cone in the middle of the caldera. Because Veniaminof is one of the highest volcanoes in Alaska at 8,225 feet, it is covered by a glacier that fills most of the caldera. Therefore, there is the possibility of a great flood of a future glacier race.

The sparsely populated Alaska peninsula runs approximately 497 miles southwest from the mainland of Alaska to the remote Aleutian Islands. Separates the Pacific Ocean from Bristol Bay, which is an arm of the Bering Sea. Small nearby settlements include Perryville (113 as of 2010), Chignik (91) and Chignik Lake (73), the vast majority of which are Native Americans.

Volcanic eruptions are a major hazard for aircraft as a result of ash columns, volcanic debris that can cause massive damage to engines. A \ W

AW-Icon-TXT-01

Lion Air JT-610 | Day 23

Lion_Air_logoBoeing will hold a press conference for airlines on the 737 MAX systems

Boeing plans to hold an online press conference with the airlines on Tuesday morning to discuss the systems of the Boeing 737 MAX model. The press conference will focus on informing and clarifying the processes of the new technology after the crash of Lion Air’s JT-610 flight.

Impact of the accident 737 MAX

The plane crash of the Boeing 737-8 MAX from the Lion Air airline in the Java Sea on October 29, 2018, had the fatality of the 189 people on board, in the first major accident that involved the fourth generation of the Boeing 737 model. The impact of the aerial tragedy has generated a strong claim about the operational safety of the most famous and prestigious aircraft in the world.

The manufacturer Boeing in an effort to bring tranquility to current and future users by the new systems incorporated to the aircraft 737 MAX, has come out to clear clouds before the crisis developed worldwide.

An accident of an Airbus A320-100 was involved in the European Airbus Group consortium when it was promoting the new A320 model that incorporated the innovative fly-by-wire system. The first A320 prototype, launched in March 1984, made the first flight on February 22, 1987.

AW-700300545.pngACCIDENT OF AIR FRANCE FLIGHT AF-296, AN AIRBUS A320-100 WITH THE NEW FLY-BY-WIRE SYSTEM

The aircraft, an Airbus A320-111, with registration F-GFKC, MSN009, flew for the first time on 6 January 1988, and was delivered to Air France on June 23, 1988. It was the third A320 delivered to Air France, the customer of launching. Air France flight AF-296 was a charter flight of a new Airbus A320-111 operated by Air France. On June 26, 1988, he crashed while making a low pass over the Mulhouse-Habsheim Airport as part of the Habsheim Airshow. Most of the shock sequence, which occurred in front of several thousand spectators, was captured on video. The cause of the accident has been the source of great controversy. This particular flight was not only the first passenger flight of the A320, it was also the first public demonstration of an aircraft with Fly-by-wire technology. It was assumed that the flyover at low speed, with the landing gear down, would take place at a height of 100 feet (33 meters); instead, the plane made the overpass to 30 feet, grazed the tops of the forest trees at the end of the runway (which had not been shown on the map of the airport that was handed to the pilots) and crashed. All passengers survived the initial impact, but one woman and two children died from smoke inhalation before they could escape.

The official reports concluded that the pilots flew too low, too slow, could not see the forest and accidentally flew towards it. The captain, Michel Asseline, disputed the report and claimed that an error in the flight computer of the Fly-by-wire systems prevented him from pushing and pulling up the command command. As a result of the accident, there were allegations that the investigators had tampered with evidence, specifically the flight recorders of the aircraft (CVR/FDR).

This was the first accident of an A320 aircraft and soon the controversy arose about the new Fly-by-wire systems were reliable for air navigation. The questioning of the innovative technological advance of Airbus put the aviation industry in jeopardy by wrapping it in a spiral with almost no exit. Fortunately the problems were corrected and at present the fly-by-wire system enjoys wide acceptance.

Conference by the Boeing 737 MAX

Boeing plans to hold a conference call with airlines on Tuesday morning to discuss systems on the 737 MAX model that crashed in Indonesia last month, according to four sources familiar with the matter.

After the accident of flight JT-610, the Federal Aviation Administration of the United States (FAA), warned airlines that erroneous entries of the sensors of the Antiblock System could cause the aircraft to sink automatically even when the autopilot is off, which makes it difficult for pilots to control.

The members of the rescue team organize the remains, showing part of the logo of Lion Air flight JT610, which crashed into the sea, at the Tanjung Priok port in Jakarta, Indonesia, on October 29, 2018. Indonesian researchers said that the system was not detailed in the Lion Air flight manual.

Boeing said last week that it had provided two updates for operators around the world that re-emphasize existing procedures to address MCAS-related situations. Boeing refused to provide further comments. On Monday, in a message to employees, Boeing CEO Dennis Muilenburg said that news reports that the manufacturer had withheld information about the MCAS from the airlines were not true and that the function had been described in the flight crew operations manual, according to aviation journalist Jon Ostrower.

Last week, American Airlines said it had ignored some features of the so-called Maneuvering Feature Enhancement System (MCAS) designed to prevent the 737 MAX from jamming.

In a memo on Friday, a United Airlines union said airline pilots were properly trained to handle a malfunction of the MCAS, although the system was not mentioned in a course for those who switched from older models to the new aircraft.

A preliminary report on the Lion Air accident will be published on November 28 or 29, 2018, according to Indonesian researchers who analyzed the aircraft’s flight data recorder. However, divers still have to locate the voice recorder in the airline cabin, which sheds light on the pilot’s interactions that are important to get a more complete picture of the circumstances of the accident.

Forensic Team

The Police Forensic Team identified three more passengers of the Lion Air JT-610 flight that crashed, raising the number of identified passengers to 104. Head of the Department of Identification of Disaster Victims (DVI), Principal Commissioner Lisda Cancer, said here on Tuesday that the forensic team has identified three more passengers. The three victims were identified through the DNA test.

Until Tuesday, the number of passengers of the unfortunate plane identified by the forensic team has reached 104, with 73 male passengers and 28 female passengers, while the remaining 85 passengers remain unaccounted for. The Boeing 737-8 MAX aircraft, with registration number PK-LQP, transported 182 passengers and 7 crews.

MOT Indonesia hardens rules

Indonesia seeks to tighten the rules on hiring foreign pilots. The Ministry of Transport (MOT) of Indonesia plans to be stricter by allowing the country’s airlines to hire foreign pilots after the recent Lion Air Boeing 737-8 MAX accident. According to Robi Ihsan, a senior MOT official in Jakarta, the authority will review the current hiring process for foreign pilots and the requirements of each airline. He said that the criteria will have to be improved depending on the type of aircraft for which the pilot will be hired, although specific details on the criteria under consideration are not yet available.

The requirement of certain Indonesian carriers for the captain’s post is only 1,000 hours of flight, while for a co-pilot it is 250 to 500 hours. The captain (CPT) of flight JT-610 was a 31-year-old Indian citizen with 6,000 flight hours. The first officer (FO), an Indonesian, was 41 years old with more than 5,000 flight hours. «We are not saying that the accident was a human error, but it is a revelation for the authorities, and we also have to make sure that the pilots with the required experience and the number of flight hours are hired for the job», Ihsan said. Robi Ihsan acknowledged that in the past there were cases of several foreign pilots who falsified reports on the registration of their flight hours. This has led airlines to analyze each entry in the pilot registration books and verify before they are hired.

The MOT officials refused to reveal how many foreign pilots were involved and where they were from. Currently, there are 20 airlines, including freighters and charter companies operating in Indonesia, a sharp contrast to the 66 in 2007 before local authorities severely confronted operators with safety oversight and ordered airlines with less than five aircraft merge or cease operations.

Indonesia is one of the fastest growing aviation markets in the world. According to estimates by local airlines, Indonesia will require some 5,300 new pilots between 2019 and 2023. The remaining 16 flight schools in the country can produce approximately 800 graduates each year. Indonesian airlines will have to resort to hiring experienced foreign pilots, that’s a global shortage problem. During a crackdown on 18 flight schools earlier this year, eight were found to be lacking in their training. Two were closed and six were told to improve.

The Lion Air accident is the second fatal accident in the country since December 2014, when an Airbus A320 AirAsia Indonesia crashed into the Java Sea shortly after takeoff from Jakarta Airport, with no survivors. A \ W

Confusión tráfico aéreo en Colombia

Image result for Aeropuerto de Palmira

Confusion air traffic in Colombia

Confusion in communications between Control Tower Cali and Lufthansa flight

The landing of a Lufthansa airline was at risk on Saturday, 24 November 2018 at Alfonso Bonilla Aragón Airport in Palmira, located between Palmira and Cali, Colombia, due to an alleged failure in communications with the control tower of the air terminal.

According to the audio released through a Facebook page, the Lufthansa aircraft would have been diverted to Bonilla Aragón because it was the alternative airport to El Dorado in Bogotá, but when the pilot tried to communicate with the tower to ask for a clue, the gap idiomatic with the controller in charge generated the confusion. In the audio, the communication between the controller, the crew of Avianca and the Lufthansa pilot is heard, and the lack of understanding between the pilot of the German airline and the controller is evident. This confusion motivates an Avianca pilot to intervene, and asks the controller to seek help from another person, because she, apparently, does not understand the English spoken by the Lufthansa pilot. The pilot of Avianca, therefore, tells him to prioritize the landing of the international plane, since that comes from a farther destination, but warns the controller that this lack of understanding could cause an accident between both aircraft. The aircraft A340 of the German airline would have been diverted by the closing of Saturday of the international airport El Dorado of Bogota due to weather conditions.

Captain Julián Pinzón, Director of Civil Aviation Safety, referred to the incident and assured that «downloading all the responsibility in one person is easy. But Cali does not have the same driver capacity that Bogotá has», he also said that the work of the controllers demand a lot of speed and that the employee of the Avianca did what was appropriate given the circumstances, highlighting that while El Dorado is a two-lane air terminal, the Cali airport has one. «She did her homework the way she is. It began to give the separation of the aircraft with heights, speeds, distances, to be able to organize the traffic and put them all. There was a drawback, and is that as the planes go to Cali as an alternate, they go with a fuel that can vary between 30 minutes and an hour. When she gave the instruction to the Lufthansa aircraft, the pilot receives the instructions but does not do what she says. The pilot at no time told him why he could not stay in manga». A\W

An Airbus A340-300 collision on platform of Ezeiza Airport

The AR1133 flight of Aerolíneas Argentinas landed this Monday at 06:58 a.m. at the Ezeiza Airport when the Airbus A340-313 from Madrid collided with the hose of a pumper on the platform. According to the Civil Aviation Accident Investigation Board (JIACC), when the aircraft performed the usual taxiing maneuvers to the platform where passengers then descended, a fire vehicle hit its left wing, causing damage to the toe . The particular case is that the truck and firefighters were in that area to honor the pilot of the Airbus A340-313, registration LV-FPU, which this morning retired with the operation that arrived from Spain.

There is a tradition in the aeronautical sector that when the commanders who leave the activity are honored with a water bridge when they make their last flight. On this occasion, what should have been a festive event almost caused a serious accident. Official sources reported that there were no injuries due to the touch of the plane with the fire truck, but it will be necessary to repair the wing before the aircraft returns to service. A \ W
