Boeing 737-200 se precipita en Hawaii
Un Boeing 737-200 de la compañía carguera Transair Rhoades Aviation, matrícula N810TA que realizaba el vuelo T4-810 de Honolulu a Kahului, en Estados Unidos con 2 tripulantes, estaba saliendo de la Pista 08R de Honolulu cuando la tripulación detuvo la subida a unos 2000 pies informando que habían perdido el motor número 1 JT8D (izquierda). La Torre de Control (TWR) despejó el vuelo para una aproximación visual a la Pista 04R. El avión comenzó a descender mientras intentaba posicionarse para regresar a la pista 04R de Honolulu. La tripulación advirtió que podrían perder su otro motor número 2, estaba funcionando muy caliente, y solicitó a la guardia costera que saliera, y solicitó girar a la derecha hacia el aeropuerto y recibió la autorización. Posteriormente, Tower despejó el avión para girar hacia una dirección de 020 para la Pista 04R, la tripulación informó que ya no tenían el aeropuerto a la vista. TWR informó que tenían una alerta de bajo nivel, la torre despejó el vuelo para aterrizar en cualquier pista, todas las operaciones en el aeropuerto se detuvieron, los servicios de emergencia fueron llamados y la guardia costera estaba en camino de salir. El avión no llegó al aeropuerto e incursionó en el Océano a unos 2,7nm de la Pista 29 del Kalaeloa (última posición recibida del transpondedor: N21.275 W158.026 a 50 pies MSL a 113 nudos sobre tierra a las 01:45L (11:45Z). Ambos tripulantes fueron rescatados, un piloto fue trasladado por aire a un hospital, el otro llevado a tierra por un bote de rescate y llevado a un hospital.
Ambos pilotos recibieron heridas graves, uno de ellos se encuentra en estado crítico y el otro en estado grave. ATC informó además a los servicios de rescate que habían perdido la comunicación con la aeronave, la aeronave había perdido un motor, no se conocía más información para ATC. La FAA confirmó que el avión desarrolló problemas en el motor y necesitó aterrizar en el agua mientras intentaba regresar a Honolulu. La guardia costera informó que la aeronave se precipitó en las aguas a unos 2 nm del Aeropuerto de Kalaeloa. Ambos tripulantes fueron llevados al Queens Medical Center, sin embargo, se desconoce su estado.
El 3 de Julio de 2021, hombres de la Guardia Costera involucrados en el rescate informaron que el campo de escombros de la aeronave tenía aproximadamente 1.25nm de largo. Descubrieron que ambos pilotos, uno (58) estaba parado en la cola de la aeronave saludando al helicóptero, el otro se agarraba a una red de carga de carga nadando alrededor de la aeronave. El piloto estaba agotado e incapaz de hablar, pero estaba consciente. El otro piloto (50) fue rescatado por un bote a las 02:51L, el piloto tenía lesiones en la cabeza y necesitaba ser limpiado de combustible de avión antes de que saliera del bote por su cuenta y fue recibido por el personal médico que lo llevó al hospital.

La NTSB ha abierto una investigación sobre el accidente que dice: «NTSB enviando un equipo de 7 investigadores para la investigación del accidente del viernes de un avión de carga Boeing 737-200 de Transair en las aguas de la isla de Oahu, cerca de Honolulu». La NTSB agregó posteriormente que un total de 10 investigadores están siendo enviados a Honolulu.
El hospital informó que uno de los pilotos (58) se encuentra en la unidad de cuidados intensivos en estado crítico, el otro (50) recibió lesiones graves en la cabeza y múltiples laceraciones y se encuentra en estado grave. Rhoades Aviation Inc. opera como Transair con cinco Boeing 737-200F (5).

Both pilots received serious injuries, one of them is in critical condition and the other in serious condition. ATC also informed the rescue services that they had lost communication with the aircraft, the aircraft had lost an engine, no further information was known to ATC. The FAA confirmed that the plane developed engine problems and needed to land in the water while trying to return to Honolulu. The coast guard reported that the aircraft dislodged in the waters about 2n meters from the Kalaeloa airport. Both crew members were taken to Queens Medical Center, however their status is unknown.
On July 3, 2021, Coast Guard men involved in the rescue reported that the debris field from the aircraft was approximately 1.25nm long. They discovered that both pilots, one (58) was standing on the tail of the aircraft waving at the helicopter, the other was holding onto a cargo net swimming around the aircraft. When the plane sank, the pilot in the tail disappeared momentarily, was seen again while staying afloat and was put into the helicopter at 02:58L. The pilot was exhausted and unable to speak, but he was conscious. The other pilot (50) was rescued by a boat at 02:51 L, the pilot had head injuries and needed to be cleaned of jet fuel before he exited the boat on his own and was greeted by medical personnel who took to the hospital.
Investigations T4-810
The NTSB has opened an investigation into the accident which reads: «NTSB sending a team of 7 investigators to investigate Friday’s crash of a Transair Boeing 737-200 cargo plane in the waters off the island of Oahu near Honolulu». The NTSB later added that a total of 10 investigators are being dispatched to Honolulu.
The hospital reported that one of the pilots (58) is in the intensive care unit in critical condition, the other (50) received severe head injuries and multiple lacerations and is in serious condition. Rhoades Aviation Inc. operates as Transair with five Boeing 737-200Fs (5).
